Instructions for Ordering

  1. Download the order form (requires Adobe Acrobat Reader to view/print)
  2. Circle the state and edition for purchase.
  3. Fill in the quantity.
  4. Calculate and fill in the cost subtotal from the CD-ROM Price List.
  5. Calculate and fill in the shipping/handling (3% of subtotal).
  6. Calculate and fill in the Grand Total by adding the subtotal and shipping.
  7. Mail the completed order form along with check or purchase order to:
