Vehicle Maintenance Record

 

Purpose: To provide a means of monitoring vehicular safety and serviceability and to provide a means of determining when a vehicle is due for scheduled or preventative maintenance, or when a vehicle requires unscheduled repairs or service.

 

The Department of EMS will keep a record of service and maintenance work, both scheduled and unscheduled, performed on all vehicles assigned to the service. The service shall also keep a record of all daily safety checks and monthly tire checks.

 

Maintenance and Service Records:

 

Maintenance records for both scheduled and unscheduled service shall be kept on file in the department for a minimum of two (2) years, and shall be available for review. In addition, businesses that contract with Department of EMS to provide vehicular maintenance and repair, shall be required to keep a complete history of each vehicles maintenance and repair services for as long as the vehicle is owned by City of Eagle Grove.

 

These records shall include, but not be limited to, the following items:

 

1.  All scheduled and preventative maintenance such as oil changes, tire replacement, transmission service, and brake service.

 

2.  All unscheduled repairs.

 

It shall be the responsibility of the Department Chief to monitor the status of the scheduled services for each of the units and to arrange to have scheduled services rendered based on individual unit mileage’s or hours.

 

Safety Checks:

 

Safety checks shall be performed on each of the vehicles assigned to the Eagle Grove Ambulance Service. The results of the safety checks shall be documented on a Daily Unit Check Form.

 

Daily safety check records shall be maintained in the department for a minimum of six (6) months. Safety checks shall be the responsibility of the Department Chief.

 

Daily safety checks of the two vehicles that will be used for emergency or routine ambulance calls, shall be performed at the start of each shift. Safety checks of transfer vehicle shall be done immediately upon return from a transfer or daily if the vehicle is not used on a transfer during the previous day.
