DESCRIPTION OF SERVICES:

• Determine financial eligibility for these programs:
  • FIP, Title XIX Medical, Food Stamps, Waiver Programs, Hawk-I Medical

• Determine financial and program eligibility for Child Care Financial assistance
• Case Management Services (no direct services are provided) for child welfare services including Protective Day Care, Family Centered Services, Wraparound, Foster Care, Emergency Shelter Care, Independent Living, and Group Residential Placements
• Case management for Waiver and State Cases
• Child Protective Assessments including investigation of child abuse
• Adult Protective investigations
• Child Day Care Registration

FEES:  No cost

ELIGIBILITY:  Varies with the program

AREA SERVED:  Wright County

TRANSPORTATION:  In emergency cases only.

ACCESSIBILITY:  Handicapped accessible.

LANGUAGES:   Interpreter as needed.  Some Spanish tapes and literature.
