HOURS: 8:00 a.m.  to 4:30 p.m.  Monday through Friday

DESCRIPTION OF SERVICES: Provides nutrition information, healthy foods, and referrals to other services. Clients are seen by a nurse and dietitian at least 1 time every 6 months.

FEES: None

ELIGIBILITY: Must live in Iowa, be pregnant, breast feeding, up to 6 months post partum or have a child or infant under age 5. Household income must be below 185% of poverty level.

AREA SERVED: Hamilton, Webster, Wright, and Humboldt Counties

APPLICATION PROCEDURE: Call each county’s public health office to enroll

TRANSPORTATION: Some counties have public transportation available

ACCESSIBILITY: ADA compliant

LANGUAGES:  A Spanish interpreter is available at the Clarion clinics
