DESCRIPTION OF SERVICES:  Supported Community Living Services for mentally challenged adults.  Services include:

• Medication monitoring

• Activities of daily living

• Daily contact

• Socialization

• Representative Payee

• Skills training

• Transportation

• Advocacy services

Supported Employment Services including selective job placement, selective placement, supported employment, and job coaching.

FEES:  Contracted by state, county, or insurance payment.

ELIGIBILITY:  Need for assistance to remain living independently in the community.

AREA SERVED:  Webster City

APPLICATION PROCEDURE:  Case Management or contact the main office.

TRANSPORTATION:  Provided when needed.

ACCESSIBILITY:  Office is ADA compliant.  Apartments for persons with disabilities are available.

LANGUAGES:  Will provide translators if needed.
