DESCRIPTION OF SERVICES:

1. Community-Based Psychosocial Evaluation: Family Assessment including; family history, interactional patterns, stresses and concerns that impede functioning; medical consideration, strengths and support systems that enhance functioning, family’s motivation to address concerns.
2. In-Home Family Treatment: Early intervention/prevention in-home services to families where one of the children are in a situation where placement to a more restrictive level of care may be necessary.
3. Family Preservation Services: Goal-directed, time-limited intensive in-home service where the primary objective is that children be maintained at home, that family relationships are strengthened, crisis is alleviated and coping skills are enhanced.
4. Groups: Clients with similar concerns, experiences, and support needs meet in a group setting at the office or in the community. The focus is to provide educational and skill-building services facilitated by a life-skills management professional counselor.

FEES: Services are purchased through the Department of Human Services, T19, private pay, or third party insurance.

ELIGIBILITY: Families experiencing problems where one child is involved with Juvenile court services, or is at risk of being placed outside of the home.

AREA SERVED: Hamilton, Humboldt, and Wright counties.

APPLICATION PROCEDURE: Referrals made by the Department of Human Services or personal referral.
