Cedar Lane Estates, Inc. Residential Program Criteria for Admission
Cedar Lane Estates, Inc. is a licensed 45 bed, Residential Care Facility (RCF). This summary is only for the residential services provided by Cedar Lane Estates, Inc.. The following is a list of services and requirements the facility must maintain to be in compliance with policies/procedures and the State of Iowa Health Care regulations under Chapters 135C, 57 and 60:
Preparation of 3 meals/day plus snacks. Individuals must eat meals independently, or if needed with some minimal assistance or cueing. Cedar Lane Estates, Inc. only offers ADA diets of the following:
w Regular and/or regular diet w Low fat / Low cholesterol w Chopped meat
w 1500 calorie w Limited concentrated sweets w Ground meat
w 1800 calorie w No added salt
Administration of oral and/or topical medications is done by Certified Medication Aides (CMAs) employed by the facility. The facility provides training to individual residents to self-administer own medication (per physician written orders to do so). The facility only uses the OPUS system to dispense medication to residents. The facility contracts pharmacy services from a Greene Countypharmacist. If the resident does not choose to use the facility's pharmacist, they can choose another pharmacist. However, the chosen pharmacist must participate in the OPUS system or there may be a charged fee to place medication in the OPUS system from the facility's designated pharmacist.
The facility is not permitted to serve individuals that have physician's written orders of the following:
· Administer insulin or any other type of injections.
· Administer oxygen from oxygen machine or oxygen tanks.
· Diets that are not within the facility's dietary guidelines.
· Resident requires nursing care or a higher level of care.
However, if the facility has a written physician's orders that the individual can self-administer their own oxygen or injections then the individual may be appropriate for the facility's level of care.
The facility provides 24 hours of supervision, encouragement or verbal prompts (cueing) residents with their ADL skills. The facility is required to have a personnel ratio of 25:1 for daytime hours, 35:1 for evening hours, and 45:1 for overnight hours. Residents are expected to make their own bed and perform own personal chores. Residents that experience some incontinence, but are able to clean self and change clothes independently are appropriate for the facility's level of care. If incontinence is not managed by the resident and requires more than partial assistance from staff, usually this can result into the resident needing a higher level of care than this facility can provide.
The facility serves a variety of disabilities, including mental retardation, physical, and chronic mental illness (controlled or no behavior problems). An individual with diagnosis of alcoholism and/or drug addiction is required to be free of abuse in the last six months.
Individuals must be ambulatory or ambulate by use of assistive devices by self (walker, quad cane, wheelchair, so on)
Persons receiving residential services must care for own urinary cath./catheter irrigation.
Persons receiving services must bathe two times a week independently or, if needed, minimal/partial assistance is available.
For persons receiving residential services under the Code of Iowa, Chapter 37, "Standards For The Care Of And Services To County Care Facility Residents with Mental Illness and Mental Retardation," Human Services [441]. The resident must participate in an organized Individual Program Planning (IPP) process. Other residents not DD/MR/MI under this chapter must also participate in an organized IPP or Care Plan.
The facility is required to provide preplanned and monthly scheduled recreational activities. This includes 45 minutes of activities, per person, per week. Residents are encouraged to participate in as many recreational activities as tolerated.
Chapter 135C.4 Code of Iowa Health Care Facilities defines Residential Care Facilities (RCF) as a licensed facility shall provide an organized continuous twenty-four hour program of care commensurate with the needs of the residents of the home and under the immediate direction of persons approved and certified by the department whose combined training and supervised experience is such as to ensure adequate and competent care. All admissions to residential care facilities shall be based on an order written by a physician certifying (Certificate of Care) that the individual being admitted does not require nursing services. All physicians orders/physicals/certification must be signed and dated by a State of Iowa licensed physician.
135C.23 (2) A health care facility shall not knowingly admit or retain a resident:
a.) Who is dangerous to the resident (self) or other residents.
b.) Who is in an acute stage of alcoholism, drug addiction, or mental illness.
c.) Whose condition or conduct is such that the resident would be unduly disturbing to other residents.
d.) Who is in need of medical procedures, as determined by a physician, or services that cannot be or are not carried out in the facility.
Code of Iowa, Chapter 57, "Residential Care Facilities," Inspections and Appeals [481].
57.13(1)a. No resident shall be admitted to or retained in a residential care facility who is in need of greater services than the facility can provide.
57.15(7) Residents shall be admitted to a residential care facility only on a written order signed by a physician certifying that the individual being admitted requires no more than personal care and supervision but does not require nursing care.
If a resident is a person with a primary diagnosis of a disability as determined by his/her physician he/she may participate in the vocational training program. This program is available through an affiliate program called "Greene CountyVocational Services." Cedar Lane Estates, Inc. also has another program called "Tower Hill," this is an adult day services for individuals living in the community or receiving residential services. Both of these affiliate programs have their own separate regulations and summary of services. Both affiliate programs is located on the grounds of Cedar Lane Estates, Inc..
Cedar Lane Estates, Inc. receives several types of reimbursements services. Most individuals pay for services through own private funds, insurance, SSA, SSI, or receive funding from their legal settled county Central Point of Coordination (CPC).
The facility also provides 24-hour respite services (up to 14 days) for individuals that meet the same requirements as a resident of Cedar Lane Estates, Inc..
