Frank Pierce (Nicholas Cage) is an EMT who is at the end of his rope. His job of transporting the dying and dead has left him hopeless, depressed, and haunted. He revels in the lives he has not been able to save and now he is seeing their ghosts. Now, he is trying to get fired from work rather than face losing another person. Unfortunately, he keeps being drawn back. He befriends Mary Burke (Patricia Arquette), the daughter of a man he tries to save. She is as down and out as he is. Together, he might begin to understand why he is being haunted and begin to save his soul.
